SUMMON DECLARATION OF VEHICLES 

According to the above, I am    ,plead that I have used the car as mentioned above such as the dates and times listed below:

2. It is my own responsibility for any summons or any legal action from Malaysia or Singapore within the date and time mentioned above. I will take full responsibility for the car as mentioned above. 
3. The summons or any legal action can be posted to me directly and I will pay for all the cost that it will take. 
4. I also agree to give a copy of my Identification Card and License for your reference. 
5. I will also agree to pay for all the damages and loss of the car within the date and time frame mentioned above to the right owner of the vehicle within  7 days working days.
